Insurance
Bring along your medical insurance card and information about your vision care plan if you have one. Learn more about insurance we accept.
Cancellations
We request 24 hours notice for cancellations so we can try to give your spot to another patient. Text or call (304) 768-3332 as soon as you know you need to reschedule.
Medications
Make sure to bring a complete and update list of your medications with you to your appointment. We can scan a list to your record and save you the trouble of listing them all out on your forms!
Current Lenses
If you currently wear glasses or contact lenses, make sure to bring or wear them to your appointment. If you have both, bring both. If you wear or are interested in wearing contact lenses, make sure to review our contact lens fitting levels of service.
No pets, please
We love animals, but for the health and safety of all of our patients we cannot accommodate pets in the office. If you have a disability and have service dog that is trained to perform a specific task for you, please discuss with us in advance so we can make special accommodations.
Directions
Our address is 1 Stewart Plaza, Dunbar, WV, 25064. We’re between the Dunbar library and Keller Funeral Home along Myers Avenue.
From 10th street turn onto Myers Avenue then right on Ohio Avenue.
From Dunbar Avenue, take13th Street, then go left on Myers Avenue, then left again on Ohio Avenue.
From our old office on Grosscup Avenue, go left on 12th Street, left on Myers Avenue, immediate right on Ohio Avenue.
Parking is available directly in front of our building or along the alley.
